Submit copy of the front and back of your ID
US PASSPORT FEE
Payable by check or money order
Made out to the US Department of State for $190.00
Confirmed travel plans
Need 2 copies of your travel plans
Applicant must be traveling in the next 17 days or 30 days if travel visa is needed.
Passport Photos
Need 2 passport photos
Must be 2x2 inches
Must not wear glasses
